Closing the gap, in an Australian first for aged care

Continuity of care is at the heart of quality aged care. Yet too often, critical medication information doesn’t move with the resident, creating gaps that can lead to delays, duplication, and preventable harm.

Residential Medication Management Reviews (RMMRs) are one of the sector’s most powerful tools for improving medication safety. But when these recommendations remain locked in emails or PDFs, they can’t reliably follow residents as they transition between aged care, hospital and home.

At Meditrax, we’ve spent over 28 years working alongside aged care providers to strengthen medication governance. We understand that real change doesn’t come from isolated fixes but from supporting the entire ecosystem we all share. That commitment has shaped our approach to this challenge.

We’ve developed Australia’s first platform to enable direct, secure upload of RMMRs to My Health Record. By making updated medication reviews visible and accessible in real time across care settings, the platform supports safer, faster, and more informed decision-making wherever a resident is in their care journey.

This system was built with direct input from aged care clinicians and consulting pharmacists, purpose-designed for aged, home and disability care. It includes offline access to suit varied facility environments, a secure cloud-based dashboard for governance and audit readiness, and seamless integration with Webstercare MedCare™ Electronic National Residential Medication Charts (eNRMC) to reduce duplication and streamline workflows.

But connection alone isn’t enough. To deliver real value, we knew it had to go beyond simply sharing information. Our focus was on giving providers a practical way to turn their medication review data into meaningful, actionable insights that genuinely support safer care.

That’s why our platform includes a purpose-built dashboard with powerful insights that enable clinical teams to see emerging trends, strengthen proactive governance and identify residents who may need urgent review. It’s about equipping pharmacists and aged care teams with timely, reliable information to support informed decisions and deliver better outcomes for residents.

Early pilot results have shown meaningful reductions in administrative workload for pharmacists, while supporting providers to strengthen medication governance and readiness for the strengthened Aged Care Strengthened Standards.
